Position Overview

The Licensed Practical Nurse (LPN/LVN) provides direct resident care and supports unit operations in collaboration with the Registered Nurse. This role delivers safe, high-quality nursing care using the nursing process in accordance with the Nurse Practice Act and facility policies, while helping ensure smooth, organized, and efficient unit operations.

Key Responsibilities

Provide direct nursing care and assist with resident care planning
Implement and monitor care using the nursing process
Delegate and supervise nursing care activities as directed
Maintain clinical competence and promote quality outcomes
Build professional relationships with residents, families, and care teams
Support regulatory compliance and documentation standards

Required Qualifications

Active LPN/LVN license in the state of employment
BLS certification from a Vibra-approved provider

Preferred Qualifications

1 year of clinical nursing experience
3 years of medical-surgical experience
1 year of specialty-specific experience
Knowledge of healthcare regulations and compliance
Strong organizational, analytical, and decision-making skills
Ability to work independently and use clinical judgment
Basic computer and EMR proficiency
