Position Overview
We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to provide one-on-one skilled nursing care to patients in their homes. The LVN works under the direction of a Registered Nurse and in collaboration with physicians and the interdisciplinary care team to deliver patient-centered care in accordance with physician orders and agency policies.
Key Responsibilities
- Provide intermittent skilled nursing care in patient residences
- Assess, monitor, document, and report patient condition and progress
- Follow individualized care plans and physician orders
- Communicate effectively with patients, families, physicians, and care team members
- Complete required clinical documentation accurately and on time using agency systems
- Support quality, safety, and compliance standards
Qualifications
- Active LVN license in the state(s) of practice
- Minimum 1 year of nursing experience preferred
- CPR certification (American Heart Association or American Red Cross)
- Strong communication and interpersonal skills
- Ability to work independently and exercise sound clinical judgment
- Commitment to quality patient care and customer service
- Willingness to participate in on-call rotation (at least once per month, as needed)
- Valid driver’s license, reliable transportation, and auto insurance required
Education
- High School Diploma or GED
- Graduate of a state-approved vocational/practical nursing program
Working Conditions & Physical Requirements
- Work performed in patient homes with frequent travel
- Requires standing, walking, kneeling, squatting, reaching, and lifting
- Ability to assist with transfers of up to 100 pounds
- Exposure to environmental conditions, body fluids, and hazardous materials
- Must meet health requirements, including annual TB testing and other state-mandated screenings
