Job Summary
The Clinical Nurse (CN) provides patient- and family-centered care in the Ambulatory Infusion Center. This role delivers evidence-based nursing care, coordinates care across the continuum, and advocates for patients and families while following nursing and medical plans of care, regulatory standards, and organizational values.
Schedule: Monday–Friday | Day Shift | 8:00 AM – 5:00 PM
Key Responsibilities
- Perform comprehensive nursing assessments, including physical, psychosocial, developmental, and educational factors
- Analyze data to determine nursing diagnoses, prioritize needs, and develop care plans
- Implement and evaluate individualized care plans according to organizational and regulatory guidelines
- Provide patient and family education on health promotion and management of acute and chronic conditions
- Coordinate care and transitions within the organization and the community to promote positive outcomes
- Advocate for patients and families, supporting access to resources and ambulatory services
- Monitor patient response to care, modify plans as needed, and document accurately in the medical record
- Ensure safe, high-quality care and compliance with regulatory and quality standards
- Supervise and delegate care activities to appropriate team members
- Support and mentor students and new staff, modeling best practices and patient-centered care
- Participate in process improvement initiatives and other duties as assigned
