Role Overview
Provide and coordinate pediatric home health services, working closely with physicians and a multidisciplinary care team. Responsibilities include home visits, patient assessments, care coordination, documentation, family education, and discharge planning.

Qualifications
- Active RN license with at least 1 year of experience
- Pediatric RN experience required (acute care preferred)
- Ability to conduct home visits in diverse environments
- Reliable transportation or access to public transit
- Strong documentation, communication, and care coordination skills
