Key Responsibilities
- Provide skilled, individualized nursing care to adult patients in their homes as ordered by the physician
- Collaborate with an interdisciplinary team to develop, implement, and update patient care plans
- Monitor patient condition, evaluate progress, and communicate findings to physicians, case managers, and families
- Administer treatments and nursing interventions based on clinical assessments
- Participate in an on-call rotation (approximately every 3–4 weeks) and complete PRN visits as needed
- Accurately complete documentation using the Kinnser (WellSky) system
- Perform additional duties as assigned
Qualifications
- Active Registered Nurse (RN) license in the state of Texas
- Minimum 1 year of RN experience (Home Health experience preferred)
- Strong verbal and written communication skills in English
- Valid Texas driver’s license and current auto liability insurance
- Current CPR/BLS certification
- Experience with Kinnser/WellSky is a plus
