Position Overview
Avem Health Partners is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join the Medical-Surgical team at Fairfax Hospital on a full-time night shift. This role provides direct and indirect patient care under the supervision of a Registered Nurse, supporting high-quality, patient-centered care in a safety-sensitive environment.
$5,000 Sign-On Bonus
Key Responsibilities
- Deliver safe, effective nursing care aligned with individualized patient care plans
- Monitor patient conditions and respond promptly to changes or treatment responses
- Assess patient needs, medical history, risk factors, and symptom urgency
- Collect and prepare specimens (blood, urine, stool, sputum) for analysis
- Administer medications according to facility policies and scope of practice
- Assist with care planning and implementation under RN direction
- Communicate clearly with physicians and interdisciplinary healthcare teams
- Maintain accurate, timely documentation in compliance with regulations
- Follow all safety, infection control, and confidentiality standards
- Respond appropriately to emergency situations
- Participate in continuing education, training, and evidence-based practice initiatives
- Foster a positive, respectful, and collaborative work environment
- Perform additional duties as assigned within scope of practice

Qualifications
Education & Licensure
- Graduate of an approved nursing program
- Active LPN license issued by the Oklahoma Board of Nursing
- BLS certification (required or obtained within 30 days of hire)
- ACLS certification preferred
Experience
- Minimum 2 years of clinical experience preferred
Physical Requirements
- Ability to lift up to 50 lbs
- Prolonged standing and walking
- Frequent pushing, pulling, lifting, reaching, and patient positioning
- Adequate visual and auditory abilities to perform clinical duties
